The School Lunch Association appreciates the cooperation of Parents/Guardians and students in understanding the importance of the completion and the timely return of the School Lunch Program Envelope.

Directions to Complete Envelope
Without this envelope your child(s) meals cannot be recorded and will not be included in our meal count for that month, which will cause delays in serving your child.

All information must be completed!
When filling out your School Lunch Program Envelope, please ensure the following:

 

Child's Name, Grade, School and Teacher are complete. Without this information it would be impossible to make contact with you to address any issues with the completion of the lunch envelope. All meals required MUST be indicated on the calendar on the front of the envelope or it may result in your child not receiving a lunch on the days you would like.

 

Steps for Completing:

Use the menu that is enclosed with your envelope to help you fill out this portion of the envelope.
All meals should be reviewed with your child in order to ensure that the menu for that day is something your child will enjoy. This will help to avoid wastage.
After review and selection you must mark the day(s) chosen on the calendar portion of the envelope with an ( X) in the box for that day. Should you wish all items for that month each day must be marked with an X. Leaving the calendar portion unmarked with a payment enclosed will cause delays in the tabulation process. Grayed out days are days that the School Lunch Program is not served due to statutory holidays.
Parent/Guardian signature is requested on the back portion of the envelope.

 

PLEASE NOTE that the School Lunch Program envelope MUST be returned to your child's school by the DATE ON THE LEFT HAND SIDE OF THE SCHOOL LUNCH PROGRAM ENVELOPE . If the envelope is not returned by this date, your child will not be able to participate in the hot, lunch program for the full month.

 

Please note that a registration form must be completed before your child can participate in the hot lunch program.
